Office Hours
Monday-Thursday 9-4 lunch 12-1 Friday 9-12
Dr. Harman does surgery cases Wednesday and Friday mornings.
Accepting New Patients
To avoid a NO SHOW or Cancelation fee you must call the office (817)774-2123 to cancel or reschedule appointments at least 24 hrs in advance .
ALL PATIENTS ON TESTOSTERONE OR ANY CONTROLLED SUBSTANCE MUST BE SEEN IN OFFICE EVERY 6 MONTHS.
Insurance Information
Insurances: Blue Cross Blue Shield, UHC, Cigna, Aetna, Multiplan, First Health, Humana & Tricare
Please call the office at (817) 774-2123 for a complete comprehensive list of accepted insurances.
Referral Required from Primary Care Physician (PCP) on all HMO insurance policies.
NO Medicare, NO Medicaid, NO Medicare Supplemental, NO Medicaid Supplemental Insurance Plans Accepted
Self-Pay patient discounts are available for those without insurance.
Surgery Information
We require at least a 10-Business Day notice be given verbally or in writing to cancel or reschedule surgery to avoid a surgery cancellation/reschedule fee of $250.00
